Good Faith Estimate (No Surprises Act)

At Value Life Mental Health Group, we believe in transparency. If you are not using insurance or are considered uninsured, you have the right to receive a Good Faith Estimate that explains the cost of your mental health services before your appointment.

What Is a Good Faith Estimate?

A Good Faith Estimate outlines the expected costs of your care. This helps you make informed decisions and avoid unexpected charges.

When Can You Expect It?

  • You will receive a GFE in writing at least 1 business day before your scheduled service.
  • You may also request a GFE at any time, even before booking.

What’s Included?

The GFE will list:

  • The types of services we expect to provide
  • The cost of each service
  • Total estimated charges

Note: The GFE is an estimate. Actual charges may vary based on your specific treatment needs.

Dispute Protection

If you receive a bill that is $400 or more above the estimate, you have the right to dispute it through a federal process.

Insurance

We accept the following insurance plans:

Quest
Magellan
Cigna
Carelon

How can i add insurances manually?

We will submit claims on your behalf. However, you are responsible for any portion not covered by your insurance (e.g., copayments, deductibles, or denied services).

If your insurance changes, please notify us as soon as possible to avoid billing issues.

Self-Pay Rates

We offer private pay options for clients not using insurance. Please contact our office for our current rates.

Payment Methods

We accept:

  • Credit and debit cards (due at the time of service)
  • HSA/FSA cards (when eligible)

A valid card is required on file for billing purposes. Charges for services, missed appointments, or late cancellations will be applied automatically as outlined in our policy.

Cancellation & No-Show Policy

To avoid a $100 missed appointment fee, please cancel or reschedule at least 24 hours in advance.

Repeated missed appointments may result in termination of care.

Financial Responsibility

Clients are responsible for all charges incurred, including those not covered by insurance. A financial agreement is signed prior to your first appointment.
